All contributions will remain in the communities in which they are given, unless instructed otherwise by the donor.
Please send your check made payable to Centrica Care Navigators. Download a donation form, then mail the completed form to Centrica Care Navigators, 7100 Stadium Drive, Kalamazoo, MI 49009.

The family requests that memorial gifts be made in support of Centrica Care Navigators (or, optionally, the name of a specific fund). Checks can be mailed to Centrica Care Navigators, 7100 Stadium Drive, Kalamazoo, MI 49009. Or, they can be made online at above. Please indicate that the gift is being made in memory of [name] by writing a memory on the check or checking the “in memory of” box when making a gift online.
If you include the name and the address of the person you would like to be notified, Centrica will gratefully share that you have made a gift in their honor or in their memory. We do not share the gift amount.
You may designate your gift to the existing fund that is most meaningful to you. Donated funds stay in the community or region where you live. Occasionally, a specific fund for memorial gifts will be designated by the family. In these cases, unless otherwise noted by the donor, gifts received will be used for the fund selected by the family.

You can designate your gift more specifically by creating a Family Fund others can give to.
Endowments are a wonderful way to honor someone and leave a personal legacy. Endowments can be created with a commitment of at least $25,000 (payable over five years) and will generate an annual impact that recognizes your honoree in perpetuity. Read more about Family Funds here.
Your gift to Centrica Care Navigators is tax-deductible. Hospice of Michigan is a not-for-profit tax-exempt organization under section 501(c)(3) of the Internal Revenue Code. Centrica Care Navigator’s Tax ID is 38-2293985. We will always provide a tax receipt upon receiving a donation.
Please give the gift of hospice and join the Caring Circle Society by making a gift each year. Your contribution allows us to continue offering hospice and palliative care, bereavement services to adults and children, and community education and outreach in our community.
