Hospice Eligibility for Dementia Patients

Eligibility is based on documented evidence of advanced dementia, including:

  • Stage 7 or beyond according to the Functional Assessment Staging Tool (FAST)
  • Unable to ambulate, dress or bathe without assistance
  • Intermittent or constant incontinence
  • Lack of consistently meaningful verbal communication (e.g., stereotypical phrases only or approximately six or fewer intelligible words per day)
  • Additionally, the patient must have had one of the following within the past 12 months: Aspiration pneumonia; pyelonephritis; septicemia; decubitus ulcers, multiple, stage 3-4; fever, recurrent after antibiotics; inability to maintain sufficient fluid and calorie intake with 10% weight loss during the previous six months or serum albumin < 2.5 gm/dl
