When is it time to call Hospice?
Hospice care is about hope that the time before the end of your life is as comfortable and meaningful as possible. Sometimes families wait to call hospice until the final days when hospice could have provided specialized services to meet all your needs and given the support to you and your family that is needed during the end of life journey. If you or your loved one is diagnosed by a physician with a terminal illness with less than six months to live if the illness follows its usual path you are eligible for hospice care. If you don’t choose to follow curative measures for your terminal illness, our hospice team is there for you. Anyone can call any hospice you wish; a doctor, family member, or friend. There are differences in hospice organizations. As you learn more about what to expect with Centrica Care Navigators, you’ll find a pervasive theme of compassionate caring.
A nurse visit is scheduled.
After the referral call, admissions begins with an assessment. You will be visited wherever you are by an RN from Centrica Hospice Care. That could be in your house, hospital, long-term care facility, wherever you call home. We accept clients based on their need for care, not their ability to pay. A doctor must determine you have six months or less to live based on the usual course of a terminal illness. Once the RN determines you meet Medicare guidelines, we work with your physician to manage any medical needs you may have.

Your attending physician directs your medical care. Your Centrica Hospice team provides for your needs.
