Letter from the CEO
Dear Friends,
I hope this message finds you in good health and spirits. As I reflect on this year, I’d like to share some remarkable milestones achieved by our agency in recent months. One milestone was reaching an all-time high in census. We are humbled and honored that so many families are entrusting us to care for their loved ones.
The dedication of our staff has propelled us to this remarkable achievement. Their unwavering commitment to providing quality care and support to our patients and families has been truly incredible. I am immensely grateful for their resilience and passion, as they have worked tirelessly to ensure each patient receives the care and attention they deserve.
Additionally, I am excited to share that Centrica Palliative Care continues to grow and allow us to provide our patients with an extra layer of support while navigating life with an advanced serious illness. Though the program is not new, the Bronson Home-Based Advanced Illness Management program seamlessly transitioned to Centrica Palliative Care at the end of 2022. Bronson has worked collaboratively with Centrica Care Navigators for many years. The transition allowed us to expand the program to not only our current Bronson patients but also to others within the communities we serve. We are pleased to have had a full year since we have been able to have our faithful volunteers back in full swing after a temporary suspension of their services due to the COVID-19 pandemic. Their invaluable contributions play an integral role in providing holistic care to our patients. From companionship to emotional support and respite for families, our volunteers make a difference in support of our families, and we are grateful to have them back in action.
None of these accomplishments would have been possible without your enduring support and belief in our mission. Your dedication to Centrica Care Navigators continues to inspire and motivate us as we strive to make a positive impact on the lives of those we serve. We are thankful for your unwavering commitment to our agency and our shared vision. Together, we can create a lasting difference in the lives of individuals and their families during their most vulnerable moments.

Being there is what counts
In October 2021, Ron Osborne’s father, Jim, went to the hospital several times. His health had been declining for a while, and after all his visits, a doctor told him that, if he chose to, he could go home — which meant going home to die.
That is what Jim Osborne wanted to do; he felt he was ready. But he didn’t have to be at home without any support. Ron and his brothers were there to help their dad, and so was the team at Centrica Care Navigators.
Ron says he and his brothers worked out a 24-hour schedule so that a family member was always available for their father. They coordinated with the hospice care specialists in the Centrica Care Navigators interdisciplinary group (IDG) to learn which services could be provided and when it would be most appropriate to receive them.
Ron says he especially appreciated how the care team did everything it could to help his dad. He remembered a late-evening delivery of something his father needed. It didn’t arrive until about 7:00 p.m., only because it was a busy day for the team, and winter weather made driving tricky.
“They were like the post office — they were going to deliver that mail, one way or another,” he says. “We got what we needed to make my dad more comfortable.”
Volunteering: How to become a better listener
Centrica Care Navigators volunteers say one of their favorite parts of serving their community is helping the patients and caregivers many of them meet every day. They like getting to know peoples’ stories and sometimes even talking about their own lives.
For long-time volunteer Kathy Koets, talking with patients and their families has taught her a little something about herself, too.
Volunteers like Kathy who provide companionship say it’s rewarding to make connections with patients and their families. Conversations can help patients stay engaged with the people around them.
There are also opportunities that aren’t one-on-one with patients, like data entry or partnering with staff at Golf ‘Fore’ Hospice, the Butterfly Release and our other special events.

What you need to know about Centrica Palliative Care
A few years ago, we changed our agency’s name to Centrica Care Navigators from Hospice Care of Southwest Michigan. It was a big change, but an important one. We did it because we want to emphasize that we provide more than just hospice care — we offer a range of services, including grief support, specialized therapies, and one you might not be familiar with yet: palliative care.
Palliative care is specialized medical care for people living with a serious illness, such as cancer or heart disease. This type of care is focused on providing relief from the symptoms and stress of the illness. Patients in palliative care continue to seek treatment for their condition.
The goal of Centrica Palliative Care, like all Centrica Care Navigators’ services, is to help improve the quality of life and lower the symptom burden for patients and their family/caregivers.
A visit to the doctor can lead to a referral to palliative care. Talk with your physician if you feel you or a loved one is right for palliative care. He or she will reach out to us and start the process of partnering with us.
Centrica Palliative Care providers will provide patients and their family/caregivers with a better understanding of the prognosis, their treatment options, and will help clarify their goals of care as the disease progresses.
The most important thing to remember is that the Centrica Care Navigators team is here to help you and your loved ones get appropriate care, no matter what form that needs to take.
