Centrica Care Navigators is in our second year of funding through the Thome Enterprise Grant.

In September 2023, we received a $288K award to respond to an increase in demand for services for individuals and caregivers coping with illness, aging, dying, and loss by increasing our staffing to meet the demand and provide compassionate care that promotes physical and mental health, social engagement, and emotional wellbeing.

With the Thome Foundation’s support and the hard work and dedication of Centrica Care Navigators’ staff and volunteers, we have increased the total number of patients and caregivers served by 8% thanks to this generous grant. 

We look forward to another successful year of partnership with the Thome Foundation serving those with advanced illness, end-of-live care needs, and grieving families throughout southwest Michigan.
